I'm helping a family member deal with identity theft and I figured this would be the perfect time to post resources for anyone else going through this ring of hell. It's very important that you report the fraud immediately at the first opportunity possible!

I'll post info here for US citizens. I think it'd be great if others would like to post info here for their countries so that it's easy to find.
